Yes, that is almost certainly a bad battery. As batteries age, the chemical reactions that generate electricity start to break down and generate gasses instead. Those gasses cause the outer coating to swell, expanding the only direction possible, out the back of the phone. If you check your battery health, I suspect you'll find it's degraded, but regardless a swelling battery needs to be replaced immediately. Battery is the first thing I would suspect before anything else. It's often the culprit. After a few years batteries start to wear down.
Be VERY careful. If it is the battery, it can explode. If you open the case, do it in a well ventilated area and keep it away from anything flammable.
