where should the flat washer be placed? It fell out before I could see where it was located

Looking at the parts diagram there are no washers used.
Did you mean the diffusion plate by any chance?

flat washers normally go on screws. If the screw has a lock washer, they go on after the lock washer.
If it is just a spacer, then you need an exploded view, that you may be able to find on a parts supplier web site.
