By the "whole assembly", are you talking about a screen with the front sensor already attached? If that's the case, then no, you definitely don't need that; the front sensor assembly alone will allow you to replace the proximity sensor.
What country are you in? Availability varies by country; here in the US they're still in stock. Keep in mind that Apple doesn't sell their parts to anyone other than certified Apple repair shops, so all of iFixit's parts are aftermarket. Granted, they do go out of their way to find quality parts, but if they don't have them in stock it doesn't help you. You can click on the notify me button and get an email when the parts are back in stock, or check out some of the larger parts sellers; there are plenty of reputable dealers out there.
Be aware that you will lose Face ID when you replace the proximity sensor, as it's paired to the logic board. If you use Apple's Self Repair program, they will allow you to run their configuration tool to pair the new part (that you have to buy from them) to your phone, thus restoring Face ID. It's unclear if the Repair Assistant, available as of iOS 18, will let you pair a genuine Apple part (new or used) and restore Face ID.
Damaged flex cables can be repaired or replaced, but it requires some very specialized skills to do so, as shown in this video.
