As laptops get old, they get hotter for three reasons: 1) There is dust preventing them from blowing cool air where it's needed. 2) The thermal paste needed to transfer heat from the hot components to the heat sinks breaks down and 3) The fans get old and die.
Your cooling app is likely just running the fans at a very high speed to make up for the dust and perhaps make up for old thermal paste. We can assume from the loud noise that they are still functional fans. By default the fans are set to a lower speed because its quieter, but they are usually capable of much higher speeds. The cooling app is just turning them way up. There are temperature apps you can get to see if the laptop is cool enough with the higher fan speeds, or if it's still overheating despite the best effort of your fans.
Ideally you want to have your laptop cleaned and have the thermal paste replaced about once every 5 years. Less if you live in a very dusty environment.
