So far this has only happened when moving very slowly, but if it were to happen when moving faster it could obviously cause a serious accident. What could be causing this? Is there some system that detects a dangerous situation (falsely in this case) and automatically applies the brakes? If so what's involved in fixing it?

Then it starts fine. What could be the cause of this?
