Fein Multitool MSXE636II 1963: How to attach sawblades so it does not loosen up?

I purchased this tool kit at an auction. when I attach a sawblade it eventually loosens. I put a rubber washer in and that seems to help it stay together. It appears from the manual that I have the correct parts. I need to see one properly assembled to verify i am not missing anything and have the correct blades (as i have seen several different types).
