Why would my battery charger indicate the voltage is too low to charge while the battery is in the car, but when I take the battery out, it charges fine on the same battery charger without showing any faults?

2007 kia sedona have a code reader. And it says bank 1 and 2 are running to lean. And p0420 catalyst converter inefficiency or something close to that. (The code is right, im unsure if i got the words right)attempted to fix what i thought could keep the check engine light from coming on. (I didnt) and theres more then a few things that could possibly be the cause of it. So any specific advice on whst it could possibly be? instead of giving me "it could be..." nd then list 3-4 different things. Please i want help and its my daily driver to work.
